Question 1: Do you fail to give close attention to details or make careless mistakes?
          I always mess up or get careless.
          I daydream a lot of the day and do not pay attention.
          Sometimes I focus and sometimes I daydream.
          I am always there, and can always focus


Question 2: Do you get bored easily and lose your attention span during tasks?
          I can not do one thing for very long, I have to keep on changing my tasks.
          I like to shift between one task and another, but I can go back and forth.
          I only lose attention when I am bored which is not often.
          I can stay focused on one task until it is finished.


Question 3: Do people have to repeat phrases to you as if you are not listening to them?
          I can never hear what people have to say to me as if they are not even talking.
          My head is in the clouds most of the time.
          I sometimes lose focus and have to have a phrase repeated.
          I hear everything that people have to say to me.


Question 4: Do you feel easily distracted and have difficulty following through on tasks?
          All of the time and it affects my life.
          I get distracted most of the time.
          Sometimes I get distracted.
          I never get distracted.


Question 5: Has there been times when you have been reluctant to engage in tasks that require a lot of mental effort..
          I get involved in many different things and can focus on all of them.
          I sometimes avoid things that require too much thinking.
          I do not have the time for things that require too much of my time.
          I never get involved in tasks that require a lot of thinking because I cannot stay focused on those tasks.


Question 6: Do you feel disorganized?
          I cannot get anything done because I am not organized.
          When I care about something, I can get organized.
          I am usually organized, but sometimes I am forgetful.
          I am always organized.


Question 7: Do you lose things for tasks or activities that you are about to do.
          Yes, I would lose my head if it was not attached to my body.
          Most of the time, but then I get back on track.
          I sometimes do, but for the most part I can keep track of things.
          I am super organized and always remember where everything is.


Question 8: Are you easily distracted by outside noise when you are trying to concentrate?
          I can never focus and always hear outside noise.
          I hear outside noise most of the time and it is hard to focus.
          I sometimes hear it and it affects what I am doing.
          I can hear outside noise and still keep on doing what I need to be doing.
